El mateix pacient \u00e9s una font essencial per aportar aquesta informaci\u00f3, ja que t\u00e9 una visi\u00f3 global de l&#8217;atenci\u00f3 rebuda en primera persona. Tanmateix, la seva perspectiva ha estat tradicionalment ignorada. A partir d&#8217;aquesta premissa, hem desenvolupat l&#8217;eina SinergiAPS (\u201cSinergies entre professionals i pacients per a una Atenci\u00f3 Prim\u00e0ria Segura\u201d).    <\/span><\/p>\n<p style=\"margin: 0cm; background: white;\"><span style=\"font-size: 10.5pt; font-family: 'Open Sans',sans-serif; color: #606166;\"> <\/span><\/p>\n<p style=\"margin: 0cm; background: white;\"><span style=\"font-size: 10.5pt; font-family: 'Open Sans',sans-serif; color: #606166;\">SinergiAPS t\u00e9 l&#8217;objectiu de millorar la seguretat del pacient als centres de salut d&#8217;Atenci\u00f3 Prim\u00e0ria, a trav\u00e9s de la perspectiva i les experi\u00e8ncies dels mateixos pacients. <\/span><\/p>\n<p style=\"background: white; margin: 13.5pt 0cm 0cm 0cm;\"><span style=\"font-size: 10.5pt; font-family: 'Open Sans',sans-serif; color: #606166;\"> SinergiAPS ha estat dissenyada conjuntament per un equip multidisciplinari, que inclou tant pacients com professionals sanitaris d&#8217;Atenci\u00f3 Prim\u00e0ria. El q\u00fcestionari es completa en l\u00ednia mitjan\u00e7ant un enlla\u00e7 espec\u00edfic per a cada centre de salut, generat per l&#8217;eina web SinergiAPS. Els centres de salut poden convidar els seus pacients a completar el q\u00fcestionari mitjan\u00e7ant correu electr\u00f2nic, SMS, o a trav\u00e9s de cartells als centres de salut amb codis QR. Tamb\u00e9 poden completar el q\u00fcestionari al mateix centre de salut, administrat pels professionals del centre. D&#8217;altra banda, vam dur a terme la <a style=\"color: #000080; text-decoration: underline;\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35188925\/\"><span style=\"text-decoration: underline; color: #000080;\">traducci\u00f3 i adaptaci\u00f3 transcultural del q\u00fcestionari PREOS-PC<\/span><\/a> (base de la intervenci\u00f3 SinergiAPS, ja que \u00e9s el q\u00fcestionari que s&#8217;utilitza per realitzar les auditories als centres de salut). Un cop desenvolupada, vam pilotar SinergiAPS en un <a style=\"color: #000080; text-decoration: underline;\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35253839\/\"><span style=\"color: #000080;\"><span style=\"text-decoration: underline;\">estudi en qu\u00e8 van participar 10 centres de salut de Mallorca<\/span><\/span><\/a>. Aquest estudi va mostrar el potencial de la intervenci\u00f3 per reduir la taxa d&#8217;hospitalitzacions evitables, que va disminuir un 42% (p=0,07), aix\u00ed com per identificar millores en la intervenci\u00f3 que es van introduir abans de la seva avaluaci\u00f3 en l&#8217;assaig cl\u00ednic. Posteriorment, SinergiAPS va ser avaluada en un assaig cl\u00ednic en qu\u00e8 van participar 59 centres de salut de Mallorca i Catalunya (12 mesos de seguiment).<\/p>\n<p> <span style=\"color: #000080;\"><a style=\"color: #000080; text-decoration: underline;\" href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT03837912?term=ricci-cabello&amp;draw=2&amp;rank=1\"><span style=\"text-decoration: underline;\">L&#8217;assaig<\/span><\/a><\/span>, que va finalitzar el passat setembre de 2021, es va veure fortament afectat per la <a style=\"color: #000080; text-decoration: underline;\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/34961582\/\"><span style=\"color: #000080;\">irrupci\u00f3 de la pand\u00e8mia per COVID-19<\/span><\/a>, de manera que els centres de salut no van poder dur a terme la intervenci\u00f3 tal com estava prevista.
